Summit360 Solutions, Inc. (“Summit360,” “we,” “us”) helps employers run benefits enrollment. Employers (our “clients”) invite their employees to book enrollment appointments with us using this scheduler. This policy explains what information the scheduler collects, how we use it, and the choices you have.
Information we collect
- When you book an appointment: your first and last name, mobile phone number, email address, how you'd like to be contacted (text, email, or both), whether you agree to receive text messages, your preferred language, and, if you choose to give them, your employee ID and the best time to reach you.
- About your appointment: the date, time, place, and status of your appointment, any changes to it, and logistics notes our staff add.
- Basic technical information: your device's time zone (so we show and send times correctly), which link you used to reach the booking page (for example, a flyer or an email), and which steps of the booking form were reached. We do not use advertising cookies or trackers.
Please don't share health information with us through the scheduler. Benefits details are discussed during your appointment.
How we use it
- To book, confirm, remind you about, change, or cancel your appointment.
- To send you appointment messages by text and/or email, as you chose.
- To staff appointments and plan enrollment for your employer.
- To improve the booking experience (for example, seeing where people get stuck).
Who we share it with
- Your employer and its benefits broker, for the purpose of your benefits enrollment (for example, whether you booked or completed an appointment).
- Service providers who run parts of the scheduler for us, such as hosting, database, email delivery, and text message delivery. They may use your information only to provide those services to us.
- When required by law, or to protect the safety of people or our services.
Text messaging: We do not sell, rent, or share your mobile phone number or your consent to receive text messages with any third parties or affiliates for their marketing or promotional purposes. Text message opt-in information and consent are never shared with third parties, except with the providers that deliver our messages.
We do not sell your personal information.
Your choices
- Reply STOP to any text to stop receiving texts from us. Reply HELP for help. See our Text Message Terms.
- Use the private link in your confirmation to change or cancel your appointment.
- Contact us (below) to ask what information we have about you, or to correct or delete it.
How we protect and keep information
Information is stored with reputable providers, sent over encrypted connections, and available only to Summit360 staff who need it for your employer's enrollment. We keep appointment records for as long as needed to support your employer's benefits enrollment and our legal obligations, then delete or anonymize them.
Children
The scheduler is for employees and is not intended for children under 16.
Changes to this policy
If we change this policy, we'll update the date at the top of this page.
